Carrie Middlebrooks, APRN, FNP-C is a board-certified Family Nurse Practitioner dedicated to providing compassionate, patient-centered care for patients ages 5 and up. With a strong foundation in evidence-based practice, Carrie focuses on promoting wellness, preventing disease, and helping patients achieve their health goals through individualized treatment plans. She earned her nursing and advanced practice education from the University of Rhode Island and her undergraduate degree from the University of Texas, Austin. She is board certified by the American Academy of Nurse Practitioners and the Pediatric Nursing Certification Board. Carrie is committed to building lasting relationships with her patients and empowering them to take an active role in their healthcare. Her clinical interests include family practice and pediatrics. Outside of her professional practice, Carrie enjoys spending time with her family.
